I've always thought that shaking the medicine bottle would cause the water molecules to align in response to the microscopic amount of the active ingredient; the smaller the amount of ingredient, the larger the aligned area of water would be around each molecule. Charge fields would be involved, I would think.

The weird thing about this is that it was discovered when a sealed vial of poisonous substance was accidentally dropped into a flask of fresh water and given to rats. They all died. Really inexplicable.
